Participant Waiver and Release

1. Participant Information & Eligibility

This release must be signed by all participants or, if the participant is under 18 years old, by their parent(s) or legal guardian(s). A parent or legal guardian must sign on behalf of any minor participant.

2. Consent to Photography & Media Use

I understand that photographs and videos of classes and events may be taken, including images of participating students. I grant Japanese Martial Arts Academy, LLC permission to use such photographs or motion pictures for business purposes, including website entries, newsletters, promotional releases, and advertising. I waive any right to inspect or approve the images or products and any right to royalties or compensation related to their use. I release Japanese Martial Arts Academy, LLC and its representatives from all claims arising from the use of such media.

3. Communication Consent

I understand that providing a valid email address and phone number is required for communication regarding classes and events. I consent to receiving emails and text messages for important updates, notifications, and announcements. Failure to provide accurate contact information may impact participation.

4. Release of Liability

I, on behalf of myself, my minor children, heirs, representatives, executors, administrators, and assigns, release, waive, discharge, and hold harmless Japanese Martial Arts Academy, LLC, its members, managers, owners, officers, employees, instructors, volunteers, agents, affiliated clubs, and other participants from any and all claims, damages, liabilities, or causes of action arising from participation in martial arts training, events, or use of facilities, whether caused in whole or in part by negligence.

5. Assumption of Risk

I voluntarily assume all risks associated with martial arts training, sports programs, and related activities, including risks of injury, permanent disability, paralysis, death, loss of property, or other social/economic losses. I acknowledge that physical contact is inherent in martial arts, and injuries may result from my own actions, the actions of others, rules of play, or conditions of equipment and premises, including risks not reasonably foreseeable at this time.

6. Health & Safety Certification

I certify that I (or my minor child) am in good physical condition and have no medical condition preventing participation. I have not been advised by any medical professional against participation.

7. Facility & Equipment Inspection

I agree to inspect facilities and equipment before participating and will notify an instructor or supervisor of any unsafe conditions. I will refuse to participate if I believe anything is unsafe. Parents/guardians of minor participants will instruct their children to comply with this rule.

8. No Refunds

There will be no refunds for entry fees due to weather, acts of nature, or failure to participate.

9. Acknowledgment & Understanding

I have read this Waiver and Release and agree to participate in anyof the programs voluntarily.
